Closes #91

Summary

  • add command UX boundaries for fails, why, explain, and ci
  • clarify command intent, default output formats, and exit-code expectations
  • preserve Stage 1/2 behavior while documenting future command scope
  • include local developer and CI usage examples

Validation

  • git diff --check
  • go test ./...
