Display and designation values should be defined in CodeSystems, not duplicated in ValueSets. Removes these values from 17 ValueSet files where they were redundantly specified for concepts from external systems (SNOMED, LOINC, NPU, etc.) and one eHealth CodeSystem that already defines them. Resolves trifork/ehealth#1289
