Update OATS spec to v1.0.0

Add v1.0.0 PDF and update README and site content to match: new
Problem Formalization section with formal system model, Attack
Lifecycle and Out of Scope subsections in the threat model, and
renumbered sections 3-14 to 4-15. Set status to Release and license
to Apache 2.0.
